When creating a Survey or Registration Form, you have many response field types to choose from.
Here is a rundown of each type of field:
Multi-line: This is great for an open response that requires a longer answer
Drop Down: This option allows users to select one answer from a set of choices. They need to click the v arrow to see the choices. Add as many answer options as you like.
Radio: This option allows users to select one option from a set of preset options. Add as many options as you like
Rating: This option allows to give an answer along a continuum. This suites questions that are intended to be answered in varying degrees. For example agree/neutral/disagree or never/sometimes/ always. Include as many answers as you like.
Checkboxes: This option allows members to select one or multiple answers.
Listboxes: This option allows users to select one or more options from a list. Keep in mind, users must hold down the command key to select multiple answers
MemberPlanet also supplies you with multiple pre-populated fields. These fields include:
Name, Phone, Date, Website URL, Address, Email, Time (this is great for scheduling appointments)
Section Dividers appear as gray blocks between questions. They can be moved to separate the form into multiple parts. Customize text to label the various portions of your form.
Description: This option allows you to add text to your form. This will not appear as a question. This is great if you want to include relevant information, or give instructions.
Required Entry: Check the Required Entry box (below ‘notes’) if you want to require the member to fill out a portion in order to submit the form. When members try to submit with a required field blank they will receive an error message
Basic Info: This gray box is pre-populated onto every form. Delete or customize it like a section divider.
Link to member info: This will pull previously stored information from the members MP profile and pre-populate the form. This is a great time saver for groups that use forms frequently.
